I can't help with the PDF, but did you know that the entire https://www.midmich.edu domain is blocked to some (and perhaps all) countries outside the US? I had to use a VPN to get a US IP address in order to access the website. I just get a timeout error if I try to access the site with an IP address in the UK or Mexico.

I created this PDF for our PTA program exams, and I think it's good to go, but I have no official training when it comes to creating anything like this. Is there a possibility that someone could look this over? I would seriously appreciate it. Any feedback or possible issues would be greatly appreciated! I created two custom buttons on the final page that sends a locked version of the PDF report to both the professor and the student. I didn't choose to completely secure the form as much as having signature verification, etc...
because I was trying to keep it as painless as possible for the faculty to use.
I have checked the form with NVDA and it works perfectly for me, but I very well could be biased and not seeing all possible issues.
